Thomas is survived by his father Frank (Rose) Kolakowski of Monroe; two brothers;Benjamin (Debbie) Kolakowski of Monroe, Michael (Elma) Kolakowski of Tucson, Arizona; a sister Laura Hoffer as well as several nieces and nephews. Sadly he was preceded in death by his mother and a sister Judy Friend.
